Skeleton: Neon-style scheduling, reduction OCC split, launch visualisations - #68

- Step graph + lowering for None/Standard overlap: OCC splits stencil kernels
into Internal/Boundary steps (Boundary on the halo stream, Internal inherits
the halo's data predecessors), vector-clock pruning of redundant syncs.
- Reductions participate in OCC via ReduceLaunchMode::{Partial,Merge}.
- Optional high-priority halo streams (Options::halo_stream_priority,
System::stream(StreamPriority)).
- export_launch_matrix_dot (streams x run() ops grid) and
export_kernel_list_dot (kernels x resource access, deps, launches).
- jacobi_nvidia: fused stencil+norm default, --unfused, --hp-halo, --dot-matrix.
- Tests: skeleton_launch_order, skeleton_reduce_split; docs + CHANGELOG.
